Problem: the gate in .github/workflows/tests.yml still says 0 (nothing measured) although tests/coverage.sh is on 19.x since pull request #1 (merge commit ad4e8fe); the fork has no COVERAGE.md because it never had the docs/coverage-baseline branch.

Diagnosis: the bootstrap threshold was a placeholder until a coverage script existed (docs/ci-cd.md section 3).

Fix: threshold 99, the lowest measured file rounded down (bin/layer-lib 183/183, bt-layer 99/100 under kcov 43); a COVERAGE.md with the measurement, the command, the merge commit and the files not measured yet. Never lowered from here (decision 0006).

Tests: the required check runs tests/coverage.sh with COVERAGE_THRESHOLD=99 on the merge ref of this pull request; locally the gate passes at 99 and fails at 100.

Serves BRIEF section 10 (tests and coverage enforced in GitHub) and org-plan section 3 step 5.
